What do I need for my upcoming visit?
Please bring your photo identification card, current insurance card, & a comprehensive list of all your current medications. Below you can find two links, one link for patient registration documents & the the other link for the HIPPA & financial documents. If you choose to, you can access these documents prior to your appointment, and print out your registration, HIPPA, and financial documents. If you complete the forms prior to your appointment, bring them with you! This can help us get you seen by the doctors sooner! When filling out the paperwork, make sure to carefully read over each section and fill it out accordingly & accurately to the best of your knowledge. First Visit
Your initial appointment will consist of a consultation. At this appointment we will listen to you to find out your main dental concerns and get an understanding of what you are looking to gain from your treatment with us. During the consultation, we will perform a comprehensive examination and provide you with treatment options that address your dental concerns. Many of the patients we see at DIAC have a complex medical history and dental situations, so sometimes it is necessary to complete a more thorough evaluation and schedule a second appointment to discuss your treatment plan.
Our goal at DIAC is to provide our patients with treatment that fits your individual needs and treatment that will have long term success. Most often, surgery is not performed the same day as the consultation.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any x-rays if applicable
- A list of medications you are presently taking
- If you have dental insurance, please bring current insurance information
